How to fill out the USPS 1583 form and get it notarized

If you want to get a virtual mailbox, P.O. box or postal mailbox in the USA you have to fill out the USPS Form 1583. In this article we’ve described why this form is needed and how to fill out it properly.

What is the USPS Form 1583?

The U.S. postal service requires all virtual mailboxes agencies or CMRA (Commercial mailing receiving agencies) to collect USPS 1583 form all customers.  This form authorized a CMRA to receive and manage mail on behalf of individuals and businesses . Customers have to fill out this form for security reasons because this way only you and authorized party will be able to access your mail.

How to complete the USPS Form 1583

First, you have to download the pre-generated copy of this form from the virtual mailbox service. Usually after the sign up process they automatically send you the form. Normally, the USPS Form 1583 divided into 3 sections: the mailbox and customer information, the CMRA information, and business information. You need to fill out information about yourself such as your full name, your current address to which your mail is forwarded to and mailbox number that you choose when you sign up for the virtual mailbox. Also, you need to write down which identification ID you will use to proof you identity.

Which identification ID you can use to notarize the Form

For the USPS Form 1583 they ask two types of identifications. One of the identifications must have photo of you. Keep in mind that they don’t accept credit cards, birth certificates, corporate IDs and SSN. Take a look at the list of documents they can accept. It’s valid for domestic and foreigners IDs.

    • Valid driver’s license, state non-driver’s identification card, U.S. Armed Forces card, Uniformed Service ID card
    • University Student ID
    • Passport, alien registration card (aka “green card”), certificate of naturalization
    • Lease, mortgage, or deed of trust
    • Voter or vehicle registration card
    • Home or vehicle insurance policy
    • Utility bill
    • Form I-94, Arrival and Departure
